This full-time, 13-week contract offers field-based opportunities to make a meaningful difference in your community. You'll be supported by a dedicated team and have access to advanced tools for patient care and documentation.
Schedule/Hours- Monday - Friday, 8:00 AM - 5:00 PM
- Current RN license (in good standing) for the state of practice
- At least 1 year of recent Home Health nursing experience; field-based knowledge preferred
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- Demonstrated experience with OASIS assessment tools
- Reliable transportation, valid driver’s license, and current auto insurance
- Complete comprehensive OASIS assessments and create individualized care plans for each patient
- Deliver skilled nursing care including wound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress, identifying and reporting changes in condition promptly to physicians and the care team
- Educate patients and their families on treatment protocols, medications, and preventive health practices
- Accurately document care using Home Care Home Base (HCHB)
- Collaborate with a multidisciplinary team- including therapists and home health aides- to coordinate high-quality care
- Participate in regular staff meetings, case conferences, and ongoing professional development
- Adhere strictly to all regulatory and agency guidelines for documentation and delivery of care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
