Physical Therapist in Home Health:
Assess and evaluate patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on assessment, determine treatment plans, perform interventions, and evaluate progress.
- Evaluate patient functional status and physical therapy needs; assist physician in developing therapy plan of care.
- Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other health care team members.
- Observe and report patient response to treatment and any changes in condition to supervising nurse and/or physician.
- Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
- Instruct and inform participating members of the health care team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Visit patients according to Plan of Treatment, complete a progress note for each visit, and submit progress notes per LHC policy.
- Complete all patient evaluations and develop the PT plan of care within state‑specific guidelines.
- Report outcomes of evaluation, goals, and anticipated projected frequency of care.
- Current Physical Therapy licensure in state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and dependable vehicle or access to public transportation.
- Ability to function in any home situation regardless of client age, race, creed, color, sex, disability, or financial condition.
$44.65 – $98.65 per visit point.
Annual total cash compensation assumes full‑time employment (40 hours per week) at full productivity. Compensation includes earnings from per visit point pay and hourly pay, determined by local labor markets, education, work experience, and performance.
