Responsibilities:- Conduct comprehensive assessments of patients' physical abilities and needs.
- Develop personalized treatment plans to enhance mobility and quality of life.
- Implement therapeutic exercises and techniques to promote recovery.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients and their families on rehabilitation processes and techniques.
- Collaborate with a multidisciplinary team to ensure holistic care.
- Document patient care and maintain accurate records in compliance with regulations.
- Provide support and encouragement to motivate patients throughout their rehabilitation journey.
- troubleshoot malfunctions and call for repairs.
- Comply with federal, state, and local legal and professional requirements by studying existing and new legislation, anticipating future legislation, enforcing adherence to requirements, advising management on the needed actions.
- Valid Physical Therapy license in the state of Kansas.
- Bachelor's degree in Physical Therapy or related field; advanced degree preferred.
- Experience in home health care settings is a plus.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate demeanor with a patient-centered approach.
- Knowledge of current physical therapy practices and techniques.
- Willingness to travel to patients' homes as needed.

Working Environment:Works indoors in Agency office and in patient homes and travels to/from patient homes.
Job Relationships:Supervised by the Therapy Program Manager and Administrator.
