Support Worker, Family Advocacy & Support Services

Position Summary

Provides direct support to families in the Family Preservation Program to promote child safety, stability, and well‑being. Works collaboratively with case managers, therapists, and community partners to ensure families receive necessary services and meet case plan goals.

• Provide daily direct and indirect support to families in home, school, and community settings
• Coordinate community resources such as healthcare, childcare, housing, employment, and benefits
• Collaborate with case managers and therapists to implement case plans
• Offer emotional support, coaching, crisis intervention, and parenting guidance
• Assist families in accessing services such as SNAP, Medicaid, SSI, and employment support
• Maintain accurate and timely documentation and prepare records for audits
• Identify and address barriers to family engagement and service success
• Transport clients as required and attend court proceedings when applicable
• Support ongoing communication between families and service providers
• Promote teamwork, cultural sensitivity, and agency values

• High School diploma or GED required
• Valid driver’s license and ability to transport clients

* Average hourly rate is $18.32 (including wage, incentives, bonuses, overtime, shift differential, etc.).
