About UsWe are a leading provider of long-term and skilled nursing facilities across Texas that is currently seeking an experienced Pharmacy Consultant to join our dedicated team of professionals serving the elderly and long-term care population.
Position SummaryAs a Long-Term Care Pharmacy Consultant, you will play a vital role in ensuring regulatory compliance, improving medication safety, and optimizing therapeutic outcomes in nursing homes. You will conduct routine drug regimen reviews (DRRs), collaborate with interdisciplinary care teams, and support facility staff with education and best practices in medication management.
Key Responsibilities- Perform monthly Medication Regimen Reviews (MRRs) for residents per CMS guidelines (F756 & F757)
- Identify and report potential medication-related problems and recommend appropriate interventions
- Maintain accurate and timely documentation in accordance with federal, state, and facility-specific requirements
- Educate nursing staff and facility leadership on medication safety, appropriate drug use, and compliance standards
- Collaborate with physicians, nurses, and other healthcare professionals to optimize pharmaceutical care
- Stay updated on changes in pharmacy regulations, CMS guidelines, and long-term care standards
- Licensed Pharmacist in the State of Texas (Required)
- Minimum of 2 years of experience in long-term care or geriatric pharmacy (Preferred)
- Knowledge of CMS regulations, particularly related to F-tags (e.g., F329, F428/F756)
- Excellent communication, critical thinking, and organizational skills
- Must be willing to travel to assigned facilities regularly (mileage reimbursement or travel stipend provided)
