Social Worker - Bachelors Non-Certified, Case Management-South

Responsibilities

• Assist in meeting the patient’s needs throughout the continuum of care.
• Guide patients, families, physicians, and the interdisciplinary team to appropriate community and adjunct resources that foster quality of life.
• Interview patients, families, caregivers, and support systems to obtain an age‑appropriate psychosocial assessment.
• Help patients, families, caregivers, and support systems adjust to illness, disabilities, and resolve difficulties within the care management process.
• Provide psychosocial assessments and develop an interdisciplinary plan of care with the patient and stakeholders to best meet the patient/family needs.
• Implement and assist with discharge planning services, particularly for complex patients.

• Bachelor’s Degree in Social Work with a current LBSW license, or must be obtained within 6 months of hire.
• Master’s Degree in Social Work preferred with LMSW or LCSW.
• One (1) year in a healthcare setting.
• One (1) year in a hospital performing medical social work preferred.
• Licensed Social Worker in the State of Texas, or must be obtained within 6 months of hire.
• Certification through the Commission for Case Management (CCM) or the American Case Management Association (ACMA) within 2 years of hire.
• Advanced communication and interpersonal skills with all levels of internal and external customers.
• Strong commitment to interdisciplinary collaboration and communication.
• Strong computer skills and proficiency with Allscripts EMR, Allscripts CM software program, IDX, and 3M within 6 months.
• Excellent human relations skills, creative problem‑solving, organization, and planning abilities.
• Ability to show initiative and work with no supervision and to tolerate frequent interruptions and demanding workload.
