Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license with reliable transportation and insurance
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience (preferred)
• Bachelor of Science in Physical Therapy from an approved program

Roles & Responsibilities:

• Conduct clinical assessments to identify patient needs and determine care appropriateness.
• Document and complete a Plan of Care (POC), progress notes, and other clinical records on an ongoing basis, and revise as necessary based on ongoing assessment data.
• Plan and implement appropriate treatment for patients with neuromuscular, neurological, and orthopedic disorders.
• Instruct patients and families in safe transfer techniques and the use of therapeutic equipment.
• Provide patient and family education regarding therapeutic procedures based on individual needs.
• Coordinate with interdisciplinary teams and oversee the performance of care by other team members when applicable.

Compliance / Notes:

• Must meet mandatory continuing education requirements.
• Applies concepts of infection control and universal precautions in patient care.
• Must read, write, and speak fluent English.
