Provides individual, family, and group counseling services to patients to address various mental health and wellness-related issues. Conducts assessments and helps patients to problem-solve areas of need. Helps to develop coping strategies and action-based goals and objectives for patients. May specialize in a particular area of counseling.

• Provide comprehensive mental health counseling services to patients of the TTUHSC School of Nursing Combest Abilene Community Health Center (ACHC) as an integral member of the interdisciplinary healthcare team.
• Conduct psychosocial assessments, evaluate mental health status, develop diagnoses, and formulate individualized treatment plans based on clinical findings.
• Deliver evidence-based behavioral health interventions and counseling services for patients referred by primary care providers.
• Provide individual and group counseling, psychoeducation, andhealth promotion activities that support patient wellness andself-management.
• Educate patients on behavioral health conditions, treatment options, community resources, and strategies to improve overall health outcomes.
• Coordinate, schedule, and facilitate the delivery of behavioralhealth services to ensure continuity of care.
• Collaborate with patients, healthcare providers, and the Interprofessional Collaborative Practice (IPCP) team to develop and implement integrated care plans.
• Maintain accurate, timely, and compliant documentation in the electronic health record (EHR) in accordance with institutional,departmental, and regulatory requirements.
• Ensure compliance with applicable licensing board regulations,TTUHSC policies, departmental procedures, and professional standards while meeting established productivity expectations.
• Communicate effectively and professionally with patients,families, and members of the healthcare team, demonstrating cultural competence and sensitivity when serving diverse populations.

• Working knowledge of patient confidentiality standards andHIPAA regulations
• Experience documenting and retrieving patient information in electronic health records (EHR)
• Experience managing patient caseloads, coordinating referrals,and completing clinical documentation within established deadlines
• Proficiency with EHR systems and Microsoft Office applications
• Experience collecting, analyzing, and reporting patient datafor grant reporting, performance improvement (PI), quality improvement (QI), and program assessment
• Bilingual in English and Spanish
• Experience obtaining and synthesizing patient psychosocial,family, and medical histories to support clinical assessment and treatment planning
