Gentiva Health Services, Inc. is seeking an After hours RN Care Manager to provide high-quality hospice care outside standard business hours. You will deliver direct patient care during nights, weekends, and on-call hours and maintain primary clinical oversight for assigned patients.
You will collaborate with physicians and the IDG to coordinate care, perform routine and death visits, document thoroughly, and educate families on hospice care and symptom management.
