Mid-Level Practitioner - Hospice

JOB SUMMARY

Advanced Nurse Practitioners will practice independently and in collaboration with Hospice Care physicians and IDT members. While providing quality patient care, will establish and maintain appropriate patient/provider relationships with a wide variety of inpatient and community residents.

Minimum Education

* Masters of Nursing

Minimum Work Experience

* 1 Years

Required Licenses/Certifications

* Accredited Advanced Nurse Practitioner Certification

* Current Texas Driver's License

* CPR

Required Skills, Knowledge, and Abilities

* Ability to carry out obligations and duties of position

* Understand the physical, psychosocial, and spiritual concerns/problems of terminally ill patients and families

* Accountable to Medical Director for clinical/medical oversight and professional services

* Demonstrated interpersonal skills

* Effectively directs personnel as appropriate

* Strong computer skills

* Research information

* Requires ability to handle confidential information

Designated Driver
