Rehabilitation Clinical Specialist

Encompass Health, a national leader in inpatient rehabilitation, is seeking a clinical rehabilitation professional to support patients recovering from major illness and injury. In this role, you will deliver evidence-based, patient-centered care; complete thorough assessments; develop and implement interdisciplinary treatment plans; coordinate with physicians, therapists, and nurses; and educate patients and families for safe discharge. Responsibilities

• Provide direct inpatient rehabilitation care to patients recovering from major illnesses and injuries
• Conduct comprehensive assessments and develop individualized treatment plans
• Coordinate interdisciplinary care with physicians, therapists, nurses, and case managers
• Monitor patient progress, adjust care plans, and document accurately in the EHREducate patients and families on recovery goals, safety, and discharge planning
• Support quality, safety, and regulatory standards within the rehab hospital
• Contribute to performance improvement initiatives and outcome tracking
• Participate in ongoing training, specialty education, and leadership development opportunities
