Licensed Practical Nurse​/Licensed Vocational Nurse

Encompass Health in Abilene, TX is hiring LPN/LVNs to provide compassionate, evidence-based care in an inpatient rehabilitation hospital. In this role, you'll assess patients, administer medications and treatments, monitor vital signs, support rehab goals, and document care in the EHR. You'll collaborate closely with RNs, therapists, and physicians to help patients recover from strokes, trauma, and major surgeries. Responsibilities

• Provide direct bedside nursing care to rehab patients under RN/physician direction
• Administer medications and treatments safely and accurately
• Monitor, record, and report patient vital signs and clinical changes
• Perform wound care, IV therapy, and other ordered procedures
• Document assessments, care, and outcomes in the EHRSupport and reinforce therapy and rehab goals in daily care
• Educate patients and families on care plans and discharge needs
• Adhere to infection control, safety, and regulatory standards
• Collaborate with the interdisciplinary team on care planning
• Participate in ongoing training and quality improvement activities
