Position: Our House Case Manager

Overview

The Case Manager is responsible for providing case management and crisis intervention to the service population, using a trauma‑informed care approach and maintaining compliance with state and federal standards.

Education and Credentials

Required:

Bachelor or undergraduate degree from an accredited university in Social Work, Sociology, Psychology, Counseling, or Criminal Justice.

Master’s degree in a social service‑related field may substitute for one year of experience.

Experience

Required:

2 years of experience providing direct case management services.

Required:

1 year of experience providing intensive in‑house‑based interventions.

Essential Functions
  • Provides intensive home‑based intervention case management services using a Trauma‑Informed Care approach to plan and document services for assigned caseload.
  • Provides crisis intervention by responding promptly and professionally to crises situations at the office, during home visits, and on phone calls within business hours using independent judgement and discretion.
  • Develops, monitors, and reviews Individual Treatment Plans designed to increase strengths, address needs, and reduce conflict in the home.
  • Provides linkages to community resources for clients to assist with enrollment in public assistance, childcare assistance, employment assistance, medical needs, and support in obtaining critical documents such as birth certificates and SS/ and transportation needs.
  • Assists in locating appropriate housing for clients upon exit from the Transitional Housing program.
  • Mandated reporter – reports any suspected abuse or neglect to the program supervisor and all appropriate state and county enforcement agencies.
  • Attends and participates in staff meetings, in‑service training, and UCAP events.
  • Participates in weekly face‑to‑face or phone supervision with the supervisor.
  • Frequent travel to assist clients with community resource connections as needed.
  • Meets all deadlines required by program supervisor and state/federal partners.
  • Measurable Deliverables
  • Meets all federal and state regulatory guidelines and standards applicable to this position.
  • Maintains a caseload in accordance with agency policies and procedures and licensing and contract standards.
  • Provides services to youth enrolled in the Transitional Housing Program.
  • Completes training and receives ongoing education on skills necessary for conflict resolution, anger management, parenting, etc.
  • Conducts follow‑up calls and/or visits to young adults in crisis situations and completes safety planning as needed.
  • Compiles, prepares, submits, and maintains accurate case records, files, forms and additional information in accordance with agency policies, licensing, and/or contract requirements.
  • Works flexible hours as needed including evenings, weekends and holidays when approved by supervisor or to resolve crisis situations when they arise.
  • Implements BCFS HHS safety protocols including evacuating with children and other staff in case of an emergency.
  • Maintains confidentiality in all areas of the service population and program operations.
  • Maintains BCFS HHS professional and ethical standards of conduct outlined in the BCFS employee policy manual, including demonstrating respect for agency staff, the service population, and community members and complying with the required dress code, at all times.
  • Credentials, Skills, and Abilities
    • Maintain licenses and certificates required, including but not limited to:
      First Aid, CPR, LMFT, LPC, LCSW, LMFT‑A, or LPC‑I.
    • Pass a pre‑employment drug screen, random drug screens, and maintain emotional control, professional composure, at all times.
    • Provide proof of work eligibility status upon request.
    • Pass a pre‑employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
    • Demonstrate the ability to:
    • Respond sensitively and competently to the service population’s cultural and socio‑economic characteristics.
    • Communicate effectively in writing and verbally.
    • Work in a fast‑paced environment.
    • Maintain computer literacy required to meet the responsibilities of the position.
