Caregiver, Home Care, Direct Support Professional​/ Direct Care

Sevita is seeking a compassionate Part-Time PM Caregiver to support individuals in their homes and communities. In this evening-based role, you'll assist with personal care, daily living activities, mobility, meal preparation, and light housekeeping while promoting dignity and independence. You'll observe and report changes in condition, follow individualized care and behavior plans, and collaborate with families and clinical teams. Responsibilities

• Provide person-centered personal care and companionship to clients in their homes
• Assist with activities of daily living such as bathing, dressing, and grooming
• Support safe mobility, transfers, and light exercise per care plans
• Monitor and report changes in clients' condition to supervisors or nurses
• Prepare simple meals and assist with feeding as needed
• Maintain a clean, safe environment through light housekeeping and organization
• Document services provided in line with agency and regulatory standards
• Follow individualized behavior or support plans as directed
• Communicate with families and care teams with empathy and professionalism
• Uphold Sevita's safety, dignity, and inclusion standards at all times
