Job Title
Ballad Health Medical Associates is currently seeking an experienced, motivated Advanced Practice Provider to join our new team of Primary Care providers in delivering home-bound patients with routine medical care — including visits related to transitions of care, annual wellness exams, and providing structure for primary care in Abingdon, Virginia.
What we're looking for:
- Ability to work independently
- Experience in care of older adult (geriatric) patients
- Previous Home care experience
- Excellent administrative, organizational, and verbal skills
- Effective communication skills, especially with older adults
- Previous EPIC experience desired but not required
- Licensure in TN and Virginia
