Center Well Home Health is seeking an LPN to provide one-on-one nursing care in patients’ homes under the supervision of an RN. You will implement care plans, educate families, and document care and outcomes to support recovery.
Qualifications include an active nursing license in the practicing state, CPR certification, a valid driver's license with vehicle insurance, and at least two years of LPN/LVN experience in a clinical setting. Home health or hospice experience is a plus.
