Registered Nurse (RN) / Licensed Practical Nurse (LPN) Position Summary
The Registered Nurse (RN) or Licensed Practical Nurse (LPN) is responsible for providing direct nursing care to residents in accordance with federal, state, and local regulations, physician orders, and facility policies. The nurse promotes the highest level of quality care while ensuring resident safety, dignity, and well-being.
Essential Duties and Responsibilities- Assess, monitor, and document residents' health status.
- Administer medications and treatments as prescribed.
- Carry out physician orders accurately and promptly.
- Monitor residents for changes in condition and notify the physician, resident representative, and nursing leadership as appropriate.
- Complete accurate and timely nursing documentation in the electronic medical record.
- Develop, update, and implement individualized care plans.
- Supervise and direct Certified Nursing Assistants (CNAs) and other assigned staff.
- Ensure completion of Point of Care (POC) documentation and assigned nursing tasks.
- Respond promptly to resident needs, emergencies, and changes in condition.
- Perform admissions, transfers, discharges, and readmissions as assigned.
- Communicate effectively with residents, families, physicians, and the interdisciplinary team.
- Maintain infection prevention and control standards.
- Follow medication administration and narcotic control procedures.
- Participate in shift report and provide accurate handoff communication.
- Ensure compliance with resident rights, confidentiality (HIPAA), and facility policies.
- Participate in mandatory education, in‑services, drills, and competency evaluations.
- Perform other duties as assigned.
- Current, unrestricted RN or LPN license in the State of New Jersey.
- Current CPR certification (or ability to obtain within required timeframe).
- Long‑term care or skilled nursing experience preferred but not required.
- Knowledge of nursing principles, medication administration, infection control, and resident care standards.
- Strong communication, organizational, and critical thinking skills.
- Ability to work independently and as part of an interdisciplinary team.
- Ability to stand, walk, bend, lift, push, and pull throughout the shift.
- Ability to lift or assist with up to 50 pounds using proper body mechanics and equipment.
- Ability to respond quickly in emergency situations.
- Ability to perform repetitive tasks including charting, medication administration, and resident care.
- Skilled Nursing Facility / Long-Term Care / Sub‑Acute setting.
- Exposure to infectious diseases and bodily fluids; appropriate PPE is provided and required.
- Requires weekend, holiday, and overtime work as needed to meet resident care needs.
