  • Provide skilled nursing care and clinical assessments in a home setting
  • Administer medications and treatments according to care plans
  • Assist with complex Activities of Daily Living (ADLs) and mobility support
  • Monitor health status, manage clinical paperwork, and provide medication reminders
    • Maintain clear, reliable communication with clients, families, and care coordinators
• What We Are Looking For:
  • Active Massachusetts LPN License (Required)
  • A compassionate, professional, and dependable work ethic
  • Previous experience in home care, assisted living, or hospital environments is a plus
  • Strong clinical communication and teamwork skills
