Overview
This role acts as the lead coordinator and supervisor for the Central Office Care Coordination Team. The position works hand-in-hand with community provider agencies, community partners, and ODMHSAS program staff to ensure seamless collaboration and support. The primary goal is to coordinate care, offer outreach and engagement services, reduce hospitalizations, crisis stays, ER visits, and enhance overall access to care for these individuals.
Key responsibilities include leading and overseeing case staffings, provider meetings, and trainings; managing data reporting and analysis; and monitoring fiscal performance to ensure services are effective, efficient, and impactful.

Level II
- Master’s degree and one year of professional experience;
- Or a bachelor’s degree and two years of professional experience;
- Or an equivalent combination of education and experience, substituting one year of professional level experience for each year of the required education
- Master’s degree and two years of professional experience;
- Or a bachelor’s degree and three years of professional experience;
- Or an equivalent combination of education and experience, substituting one year of professional level experience for each year of the required education
Preference may be given to candidates who are fully licensed, and who have previous supervisory experience.
