The Occupational Therapist (OT) effectively screens and evaluates patients with cognitive and/or activities of daily living (ADL) physical functioning disorders and develops appropriate plans of care adhering to all regulatory and clinical practice standards. Utilizing standardized assessments and evidence-based practices, the OT provides rehabilitative, skilled, and medically necessary treatment interventions for patients as part of their rehabilitation plan.
Responsibilities- Assess and evaluate patients’ cognitive and ADL physical functioning disorders.
- Develop and implement comprehensive care plans according to regulatory requirements.
- Administer various rehabilitation procedures, including ADL retraining, therapeutic activities, and exercises.
- Continuously assess patient progress and modify treatment plans as necessary.
- Provide consultation and counseling to patients, families, caregivers, and service providers.
- Document all treatment sessions and maintain patient records in compliance with company policy.
- Participate in patient care conferences and required training sessions.
- Supervise and support occupational therapy assistants, rehabilitation aides, and students.

Required Experience / Certifications / Licensure- 1+ years of Occupational Therapy experience (required).
- Long-term care or rehabilitation experience (required).
- Active Occupational Therapy license (required).
- New graduates considered if they have a Level II experience.
