The Physical Therapist will effectively screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care, developing appropriate plans of care in compliance with all regulatory and clinical practice standards. Utilizing standardized assessments and evidence-based practices, the therapist will administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
Responsibilities- Assess patient progress and adjust treatment plans in compliance with regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness and modify services accordingly.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
- Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
- Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.
- Attend required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.
- Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed.
- Adhere to APTA and state-required practice acts, codes of conduct, and ethics during professional practice.
- Minimum of 1 year of physical therapy experience; new graduates are welcome if they have Level II clinical experience.
- Long‑term care (LTC) or rehabilitation experience is required.
- Current Oklahoma state license is required.
This is a 8 hour/day, days shift position.
