- Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs
- Develop and implement individualized plans of care following clinical standards and regulations
- Administer therapeutic interventions including manual techniques, ambulation, exercises, modalities, and assistive devices
- Assess patient progress and modify treatment plans accordingly
- Document all patient interactions and treatment outcomes in electronic health records
- Communicate effectively with healthcare team members, patients, and families
- Support supervision of physical therapist assistants and rehabilitation aides
- Participate in multidisciplinary team meetings and ongoing training
- State licensure as a Physical Therapist in Oklahoma
- At least 1 year of relevant experience in Long Term Care or similar settings
- Strong clinical skills in rehabilitation techniques and wound care
- Excellent communication and documentation skills
- Ability to work independently and as part of a team
