• Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs
• Develop and implement individualized plans of care following clinical standards and regulations
• Administer therapeutic interventions including manual techniques, ambulation, exercises, modalities, and assistive devices
• Assess patient progress and modify treatment plans accordingly
• Document all patient interactions and treatment outcomes in electronic health records
• Communicate effectively with healthcare team members, patients, and families
• Support supervision of physical therapist assistants and rehabilitation aides
• Participate in multidisciplinary team meetings and ongoing training

• State licensure as a Physical Therapist in Oklahoma
• At least 1 year of relevant experience in Long Term Care or similar settings
• Strong clinical skills in rehabilitation techniques and wound care
• Excellent communication and documentation skills
• Ability to work independently and as part of a team
