Occupational Therapist
Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment interventions to patients. Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, and therapeutic exercises.
Assesses patients' progress and adjusts treatments according to clinical records. Evaluates treatment outcomes for effectiveness and modifies services in relation to patient evaluations. Provides consultations and counseling to patients, families, caregivers, and other service providers. Effectively communicates with supervisors and team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided as per regulatory and clinical requirements.
Documents all required information in the electronic documentation system and maintains patient records. Accurately records treatment minutes for services provided as per regulatory and clinical requirements. Participates in required meetings, including patient care conferences and rehabilitation meetings. Engages in facility and company-required trainings, in-services, and conferences. Able to obtain necessary DME or supplies as per regulatory and clinical practice requirements.
Supervises and supports occupational therapist assistants, rehabilitation aides, and/or students. Adheres to required practice acts, code of conduct, and ethics during professional practice as an Occupational Therapist.
