Requirements:

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice.
• Experience in skilled nursing facility or geriatric rehabilitation preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work both independently and as part of an interdisciplinary team.

• Evaluate residents' mobility, balance, strength, gait, and functional limitations through initial and ongoing assessments.
• Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability.
• Provide direct therapy interventions, including therapeutic exercises, gait training, and balance retraining.
• Monitor resident progress and adjust care plans to ensure achievement of therapy goals.
• Participate in interdisciplinary care team meetings and contribute to discharge planning.
• Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
• Supervise and guide Physical Therapist Assistants and therapy aides as necessary.
• Accurately document treatment sessions, progress notes, and billing information in a timely manner.
