Description

We are currently looking for a Full Time Clinic Coordinator to join our Residential Treatment Section team located at the Nittak Himitta Womens and Childrens Recovery Center working 8 a.m. to 5 p.m.

Summary:

Functions as a member of an interdisciplinary team to provide care coordination for potential and actual residents of the facility. The Clinic Coordinator is responsible for the screening/intake/referral process and specific administrative support activities within the facility. Serves as direct point of contact assuring that all required recording, documentation, and tracking are processed responsively, and assessments, intakes, and referrals are processed in a timely manner and in accordance with admittance protocol and procedures.

Assists program clinicians and Recovery Specialists in the facilitation of consumer orientation of the program. Assists in and performs planning, coordination and supervision of comprehensive activities program and events, including planning, scheduling and implementing recreational, daily living skills, cultural, health and wellness, arts, physical, and spiritual activities for residents.

Responsibilities:

  • Receives and processes all incoming referrals
  • Collects consumer information for screening/assessment and effectively communicate with program staff regarding referral/placement needs
  • Ensures all pre-admission requirements are fulfilled prior to scheduling clinical assessments with clinicians. Coordinate appointment schedules with clinicians and legal guardian/consumer
  • Administers approved screening and/or assessments as determined by clinical staff
  • Provides program orientation to consumers and their families
  • Assists in the coordination and completion of professionally directed psycho-educational group services for consumers and their family members under the direction of the Program Manager/licensed clinician
  • Assists in consumer discharge process as appropriate
  • Prepares narrative and statistical information monthly as required or upon request of leadership
  • Connects consumers with resources including, but not limited to, supportive services to supplement treatment and counseling as well as higher or lower levels of care and assist them in securing appointments with support services and agencies, as needed
  • Plans, evaluates and coordinates classes and activities including arranging for facilitators, scheduling facilities, and scheduling outings
  • In collaboration with program manager, formulates and organizes program plans and schedules for special events and seasonal activities
  • May accompany consumer on outings or appointments
  • Provides first class customer service to both internal and external customers and responds to inquiries, compliments, and/or complaints with professionalism, courtesy, and efficiency
  • Completes all training and development requirements and participates in continuing education opportunities
  • Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types
    - Maintains a clean, organized work area and ensures a safe environment
    - Maintains and promotes professionalism
    - Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics
    - Adheres to Chickasaw Nation dress code standards
  • Maintains knowledge of and follows all rules, regulations, policies and guidelines for assigned area
  • Performs all other duties as assigned
