Overview

We are seeking a compassionate and dependable Home Health Aide to provide personal care and support services to patients in their homes. This role assists with daily living activities, supports a safe environment, and works closely with patients, caregivers, and the nursing team. Work is performed under the supervision of a Registered Nurse.

Key Responsibilities
  • Assist patients with activities of daily living, including bathing, grooming, nutrition, and mobility.
  • Support safe transfers and ambulation.
  • Maintain a clean, safe, and comfortable environment.
  • Report changes in patient condition to supervising RN
  • Maintain a clean, safe, and comfortable environment
  • Communicate respectfully and effectively with patients, families, and staff.
  • Use and handle durable medical equipment as needed.
  • Maintain accurate, timely documentation of care.
  • Follow infection control and safety procedures
  • Uphold agency policies and professional standards.
Minimum Requirements
  • High school diploma preferred.
  • Completion of an approved training and competency Nurse Aide program.
  • Current state certification as a Nurse Aide (TX), Home Health Aide (OK)
  • Current CPR certification.
  • Reliable transportation, valid driver’s license, and current auto insurance.
  • At least one year of experience as a Hospice Aide, Nursing Assistant, or similar role preferred.
Work Environment
  • Care provided in patient homes under varying conditions.
  • Exposure to infectious diseases, blood, and bodily fluids possible.
  • Requires local travel and ability to work a flexible schedule.
  • May involve prolonged standing, walking, and lifting up to 50 lbs.
