Job description:
The family nurse practitioner is an essential role on the primary care team and plays a vital role in delivering care to the communities we serve.
Duties and Responsibilities:
- As a family medicine NP provides health care services on site and via telehealth to patients under the general direction of a licensed Physician if NP does not have full practice authority
- Provides quality care to assigned patients that is consistent with evidence-based guidelines and treatment protocols.
- Demonstrates clinical expertise through direct and indirect patient care.
- Investigates clinical problems and offers sound medical solutions.
- Contributes to the development, maintenance, and improvement of the healthcare delivery system.
- Communicates effectively with other healthcare team members, patients, parents, and significant others, tailoring the approach to their level of comprehension and understanding.
- Demonstrates the knowledge and skills necessary to provide age-appropriate care by identifying medical, psychosocial, and developmental needs and recommending appropriate interventions accordingly.
- Performs procedures utilizing medically approved approaches and equipment appropriate to the patient's age and size.
- Provides information and instructions at a level of understanding appropriate to the patient's and/or significant others' comprehension level.
- Actively participates in Quality Improvement activities.
Qualifications:
Master's in nursing with certification as a family nurse practitioner.
- Current CPR Certification Required and maintained.
- Current Vaccinations required
- Current Illinois Registration and Advanced Practice Licensure.
- DEA and controlled substance license required
- Minimum of 5 years clinical experience
- Bilingual in Spanish - Required
- Must have good verbal and written communication skills
- Must be able to respond appropriately in emergencies, must demonstrate flexibility, leadership and teamwork ability
- Must have basic computer skills (prior EHR experience)
