Home Health Aide, Healthcare Nursing, Patient Care Technician
Job Summary
The Home Health Aide provides personal healthcare and related services to clients in their home, assists with maintaining a safe and clean environment, works cooperatively with patients and families, and shares observations and concerns with the supervisor. The HHA works under the direction and supervision of a registered nurse, promoting patients' potential during their home health episode and collaborating with physicians, the clinical team, and patients in preparation for discharge.

Qualifications- Completion of a Certified Home Health Aide Program.
- At least 1 year of Home Health Aide experience in a home health agency.
- Reliable transportation with valid and current auto liability insurance.
- Good communication skills to relate professionally and positively to patients, caregivers, and the clinical team.
- Ability to read, write, and speak English.
- Demonstrates interest in the welfare of the ill and elderly.
- Proof of current CPR and hepatitis vaccinations.
- Completion of the Aide Competency Evaluation Skills Checklist and Written Skills Test.
- Provide direct patient care under the direction of the RN and according to the Aide Plan of Care.
- Assist patients with safe transfers and ambulation per agency policy.
- Report changes and document pertinent information to ensure continuity of care.
- Practice accepted infection control principles and maintain a clean, safe, and comfortable environment.
- Promote positive, respectful communication with patients, families, and staff.
- Respect patients' privacy and property.
- Assist patients with proper nutrition and adequate fluid intake.
- Contribute to the efficient operation of the agency and demonstrate effective time management.
- Perform treatments and procedures according to agency policy.
- Attend 12 hours of in‑services per year to demonstrate professional growth and competency.
- Promote the agency philosophy and administrative policies to ensure quality of care.
- Record clinical documentation in the medical record.
- Participate in weekly case conferences and interdisciplinary communication to optimize service delivery and patient progress.
- Ensure patient safety.
