Job Summary:

/ppThe Certified Nursing Assistant (CNA) provides personal care and supportive services to hospice patients in their place of residence in accordance with the patients plan of care. The CNA works under the supervision of a nurse and supports patients with activities of daily living while promoting patient comfort, dignity, and safety. The CNA plays an important role in supporting hospice patients and families by assisting with daily care needs and reporting changes in patient condition to the clinical team./ppThis

Qualifications:

Current state certification as a Certified Nursing Assistant (CNA) required/lili Hospice, home health, long-term care, or healthcare experience preferred/lili Strong communication and patient care skills/lili Ability to follow patient care plans and instructions from nursing staff/lili Ability to work independently in a home or facility setting/lili Reliable transportation, valid drivers license, and current auto liability insurance/lili Ability to read, write, and communicate effectively in English/li/ulh3

Essential Functions:

Provide personal care services including bathing, grooming, dressing, toileting, and mobility assistance/lili Assist patients with activities of daily living (ADLs) according to the plan of care/lili Document patient care and observations accurately and timely/lili Report changes in patient condition or environment to supervising nurse/lili Promote patient safety, comfort, dignity, and independence/lili Maintain a clean and safe patient environment/lili Follow infection control and safety procedures in all patient care activities/lili Assist with safe transfers and mobility using proper lifting techniques/lili Attend in-service training and maintain CNA certification requirements/lili Communicate effectively with patients, families, nurses, and interdisciplinary team members/lili Follow company policies, procedures, and patient care standards/lili Perform other related duties as assigned/li/ulp Excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package./p/div
