Description

Registered Nurse (RN) - Home Health Baylor Nurse

JOB SUMMARY : A Home Health RN Baylor Nurse performs Home Health admissions, performing OASIS-D1 start of care assessments and administering skilled nursing care to patients on an intermittent basis in their home. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.

Qualifications
  • Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
  • Three (3) years of home health nursing experience, preferred.
  • Acceptance of philosophy and goals of this Agency.
  • Ability to exercise initiative and independent judgment.
Responsibilities
  • Perform OASIS Start of Care admissions assessments.
  • Make the initial evaluation visit and regularly reevaluate the patient’s nursing needs.
  • Initiate the plan of care and necessary revisions.
  • Provide those services requiring substantial specialized nursing skills.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Prepare clinical and progress notes for each patient visit and summaries of care conferences on patients in a timely manner as per Agency policy.
  • Coordinate services.
  • Inform personnel of changes in the condition and needs of the patient.
  • Counsel the patient and family/significant others in meeting nursing and related needs.
  • Participate in and present in-service programs.
  • Understand and adhere to established Agency policies and procedures.
  • Process orders and notify physicians of patient needs and changes in condition. Complete certification/recertification orders and discharge summaries.
  • Determine the amount and type of nursing needed by each individual patient.
  • Refer to Physical Therapist, Speech‑Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
  • Supervise and teach other nursing personnel.
  • Conduct patient care conferences on patients assigned to your care.
  • Participate in peer review and Quality Assessment and Performance Improvement as assigned.
  • Give total patient care as needed.
  • Takes on‑call duty nights, weekends, and holidays, as assigned.
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency defined time frames.
  • Appropriately utilize ICD‑10 codes.
Working Environment

Works indoors in Agency office and patient homes and travels to/from patient homes.
