Team Health is seeking a quality-driven family nurse practitioner or physician assistant to join our rural health clinic in Lenawee County, Michigan. This full‑time opportunity (5 days per week) offers a consistent schedule, clinical autonomy within a collaborative care model, and the fulfillment of serving a close‑knit rural community.

About the Role

In this role, you will provide comprehensive primary care services across the lifespan, manage acute and chronic conditions, emphasize preventive care, and build long‑term relationships with patients and families. You will enjoy clinical autonomy within a collaborative care model supported by Team Health’s experienced clinical and operational leadership.

Compensation is an estimated salary range of $125,000–$131,000 annually with uncapped productivity‑based earning potential.

Key Responsibilities
  • Perform comprehensive patient evaluations, including histories and physical exams
  • Diagnose and manage acute and chronic medical conditions
  • Deliver preventive care and routine health maintenance
  • Develop, implement, and adjust individualized treatment plans
  • Provide care to patients of all ages, including pediatric and infant populations
  • Educate patients and families on wellness, disease prevention, and chronic care management
  • Maintain accurate, timely, and compliant clinical documentation
Qualifications
  • Board‑certified FNP or PA, active Michigan license (or eligibility) and DEA
  • 2+ years of clinical experience in a primary care setting preferred
  • Ability and willingness to provide care to patients of all ages, including infants
  • Strong clinical judgment with a patient‑centered approach
  • Excellent communication and collaboration skills
  • Interest in rural or community‑based healthcare
