Lenawee Community Mental Health Authority (LCMHA) in Adrian, Michigan is seeking a full‑time Intensive Care Coordinator with Wraparound (ICCW).

A Day in the Life of an ICCWAs an ICCW, you will support families of referred youth by navigating systems, identifying gaps, and connecting them to community partners. Your role includes:
- Assisting families in identifying supports, building a team, and designing a strength‑based wraparound plan.
- Coordinating services and inter‑agency resources, creating new supports when existing ones are lacking.
- Monitoring provider progress toward outcomes, facilitating transition strategies, and ensuring effective use of Community Team funds.
- Managing data required for consumer satisfaction, measurable outcomes, and resource utilization.
- Preferred Bachelor’s degree in Social Work or a related human services field.
- Licensed Michigan social worker (BSW) or equivalent credentials.
- Minimum 2 years of post‑graduate experience with children and families.
- Basic computer skills in Microsoft Word, Excel, and PowerPoint.
- Valid driver’s license.
- Strong communication skills, discretion, creativity, flexibility, and motivation.
Annualized salary range: $53,976 – $69,810 (based on 37.5‑hour week) and increases with length of service.
- Paid leave beginning on the first day, with accrual increasing after 1, 5, and 10 years.
- Annual scheduled salary increases after one year of employment.
- 15 paid holidays annually, effective from the first day of employment.

- Dental and vision coverage fully paid by the agency; medical coverage partially covered.
- Retirement benefits commencing from the first day.
