MDS Coordinator; LPN

Agawam West Rehab & Nursing is seeking an experienced LPN to join our team full-time as an MDS Coordinator
.

This salaried position is responsible for coordinating resident assessments, supporting care planning initiatives, and helping ensure accurate reimbursement and regulatory compliance. The ideal candidate is organized, detail-oriented, and knowledgeable in the MDS process within a skilled nursing environment.

• Coordinate and complete MDS assessments and related documentation
• Ensure assessments are completed accurately and submitted timely
• Collaborate with nursing, therapy, and interdisciplinary team members
• Participate in resident care planning and assessment reviews
• Support reimbursement and regulatory compliance through accurate documentation
• Maintain current knowledge of MDS and CMS guidelines

• Active Massachusetts LPN license required
• Previous MDS experience in a skilled nursing facility preferred
• Understanding of MDS scheduling, coordination, and completion
• Long‑term care or rehabilitation experience preferred
• Strong organizational and communication skills

• Salary: $91,000 - $95,000 annually (DOE)
• Quality Measure Bonus: Up to $7,500 per year
