Licensed Practical Nurse (LPN)

Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures. Uses the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Requires home visits and periodic office meetings. Must be willing to travel up to 75 miles from the home clinic.

Mileage is reimbursed at the IRS standard rate. Responsible for accepting every patient on the schedule regardless of distance. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation that impacts billing and follow-up care may lead to cancellation. No guaranteed hours with this contract. Clinician must hit weekly productivity points. Expectation of 30 points per week, which equates to approximately 40 hours of work.

Must be proficient in Home Care Home Base and have used it within the last six months. Candidates must be willing to float to other clinics if patient census is low.

Experience:

Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on specialty. License:
Must have license in hand.

Certifications:

CPR, BLS Must-Have:

- Must have recent specialty experience in home health.

- Must have reliable transportation and a current driver's license.

- Must have agency-required liability insurance.
