Admissions Coordinator – Skilled Nursing

Agawam Rehab & Nursing Campus is seeking a full-time Admissions Coordinator to join our Admissions team. Working alongside experienced admissions professionals and reporting to the Regional Admissions Director, you'll help coordinate resident admissions, communicate with referral sources, and ensure a smooth, welcoming experience for residents and families across our skilled nursing campus.

• Coordinate the admissions process from referral through resident arrival
• Communicate with hospitals, referral sources, residents, families, and the interdisciplinary team
• Collect and verify insurance information and admission documentation
• Meet with residents and families to complete the admissions process
• Maintain accurate records and data within facility systems
• Help ensure timely admissions and a positive experience for residents and families

• Experience in admissions, patient access, case management, discharge planning, healthcare coordination, or skilled nursing preferred
• Knowledge of post-acute care, rehabilitation, insurance verification, Medicare, or Medicaid preferred
• Strong communication, organizational, and customer service skills
• Ability to prioritize multiple tasks in a fast-paced healthcare environment
• Skilled nursing or long-term care experience is a plus

• $45,000-$50,000 annually, based on experience
