Program Plan Coordinator/Training and Support Services

Qualifications:
  • Bachelor’s Degree in Human Services or related field, plus one (1) year’s experience in a social services field.
  • Must be supported by three (3) or more work‑related references.
  • Must have a valid driver’s license and a good driving record, as determined by Board policy, and a car for use on the job.
Primary Responsibility:

Coordination of the development and implementation of individual day service plans as required by applicable standards/regulations in accordance with the expressed interests and identified needs of persons receiving supports.

Specific Responsibilities:
  • Focus on the person and demonstrate an understanding that his/her role in delivering supports requires flexibility, creativity, and commitment.
  • Assist people receiving supports in choosing where they will work.
  • Assist individuals supported in understanding their options and the consequences of these options as they relate to their health and emotional well‑being.
  • Solicit and honor the choices and preferences of those receiving supports.
  • Understand, respect, and protect the confidentiality and privacy of individuals being supported.
  • Interact with people in a respectful manner.
  • Honor the personality, preferences, culture, and gifts of people who cannot speak by utilizing other ways of understanding them.
  • Assist individuals in the pursuit of lifelong learning and growth.
  • Develop relationships with those receiving support that are respectful, based on mutual trust, and that maintain professional boundaries.
  • Be attentive to the health, safety, and emotional well‑being of individuals served and work with the individual and his/her support network to identify areas of risk and to create safeguards specific to these concerns.
  • Be vigilant in identifying and reporting any situation in which individuals supported by the agency are at risk of or are suspected of having encountered abuse, neglect, exploitation, or harm.
  • Consistently address challenging behaviors proactively, respectfully, and by avoiding the use of aversive or deprivation intervention techniques. Where these techniques are included in an approved support plan, ensure implementation is as specified, work to find alternatives, and advocate for the eventual elimination of these techniques from the person’s plan.
  • Coordinate and/or conduct initial evaluation of consumers vocational, academic, and social skills and prepare an annual evaluation report based on Day Services Assessment and other evaluation tools as appropriate.
  • Develop and/or conduct the implementation of written day services plans and/or behavior support plans according to policy.
  • Serve as chairperson of Interdisciplinary Team, as frequently as directed by policy, for each consumer in the development of an individual plan to address training needs in the areas of vocational skills, psychosocial needs, and daily living skills.
  • Serve as liaison between the consumer and his/her family as well as all personnel in the development and implementation of the individual’s day services plan.
  • Provide direct counseling to consumers and/or training to direct support staff in the consistent implementation of day services plans and behavior support plans.
  • Monitor direct support staff for quality of work and use of appropriate training techniques.
  • Tabulate and document monthly training data as prepared by direct support staff to determine need for program revision and/or continuation and to ensure accuracy of information.
  • Develop/implement training objectives and revise same in accordance with Department of Disabilities and Special Needs program standards.
  • Complete monthly progress reports and provide updates to Service Coordinators and Coordinators of other services as applicable.
  • Respond to and intervene with behavioral incidences.
  • Attend various program, agency, and Board Committee meetings to present programmatic information as required.
  • Transport consumers as needed in agency or personal vehicle.
  • Perform other related duties as assigned.
