Case Manager

Aiken Regional Medical Centers is seeking a Case Manager to coordinate comprehensive care for patients across emergency, surgical, mental health, and rehabilitation services. In this role, you will assess needs, develop individualized care plans, and collaborate with an interdisciplinary team to support safe transitions and positive outcomes. You'll connect patients and families with community resources, address social determinants of health, and ensure thorough documentation.

Responsibilities

• Assess patient needs and develop individualized care plans for diverse medical and behavioral health conditions.
• Coordinate services across departments, community agencies, and external providers to ensure continuity of care.
• Monitor patient progress, adjust care plans, and advocate for appropriate clinical and social resources.
• Educate patients and families on treatment options, discharge plans, and community support services.
• Document assessments, care plans, and outcomes in compliance with hospital policies and regulatory standards.
• Collaborate with physicians, nurses, therapists, and social workers to support safe transitions of care.
• Participate in interdisciplinary team meetings and case conferences to optimize patient outcomes.
• Identify barriers to care, including social determinants of health, and connect patients to support programs.
