We’re seeking a detail-oriented and compassionate Central Intake Coordinator to manage the pre-admission process and ensure proper placement of residents. This is a fully a remove role. If you’re organized, collaborative, and experienced in long-term care, we’d love to hear from you!
Job Duties:
Gather and analyze pre-admission data to determine level of care
Ensure facility can meet clinical needs before admission
Collect accurate information for all payer sources
Communicate effectively with discharge planners and referral sources
Maintain organized and confidential admission records
Follow admission and transfer policies
Collaborate with department supervisors and administrative staff
Use facility software systems for documentation and tracking
Qualifications:
High school diploma required; additional training/experience in long-term care preferred
Strong understanding of the pre-admission process
Ability to collect and evaluate clinical and financial data
Professional appearance and punctuality
Cooperative and team-oriented approach
