As an Admissions Coordinator/Liaison in our nursing home, you will play a vital role in ensuring a seamless and efficient admissions process for our patients. You will serve as a liaison between our facility, patients, families, and healthcare providers, providing exceptional care and support throughout the admissions journey.

• Coordinate the intake process, ensuring all necessary documentation and information are collected and accurately processed
• Communicate with patients, families, and healthcare providers to gather information and provide updates on admission status
• Facilitate the transfer of patient records and information to our facility
• Develop and maintain relationships with healthcare providers and community partners to ensure timely and efficient admission processes
• Provide exceptional customer service, addressing patient and family concerns, and ensuring a positive experience during the admissions process
• Collaborate with the Admissions team to ensure compliance with regulatory requirements and organizational policies
• Participate in process improvements and quality initiatives to enhance the admissions experience

• Prior experience as Admissions Coordinator in a nursing home
