Duties and Responsibilities:- Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
- Manage case assignments, draft service plans, and review case progress regularly.
- Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
- Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met.
- Facilitate clients in achieving personal independence and assist in developing daily living skills.
- Perform other duties as assigned.
- Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates.
This position does not supervise any staff.
Supervision Received:The SUD Case Manager reports to their assigned Operations Managers.
Qualifications:- Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals.
- Knowledge of behavioral health practices and community resources.
- Ability to work independently and as part of a team.
- High school diploma or equivalent is required;
- a college degree in Human Services is preferred.
- CPR certification is preferred.
- NPI number/Ohio Medicaid required (instructions on how to apply will be provided).
- CDCA Certification is required.
- Minimum of 3 years in behavioral health or a related field is preferred, but not mandatory.
- Requires sitting for extended periods and occasional lifting of up to 20 pounds.
- Must pass all Federal and State background checks according to company policy.
- Flexible scheduling options allow for accommodating personal availability, enabling as little or as much work as desired.
