SummaryThe Home Health Care Aide provides personal care, assists with food preparation, assists with exercises, and provides other activities of daily living as ordered by the physician.
Responsibilities- Assists, prompts, and/or provides direct personal care and assists with food preparation and other activities of daily living (ADLs) in order to maintain proper hygiene and nutrition for select patients.
- Performs simple procedures as an extension of therapy or nursing services as indicated in the patient plan of care; may assist in administering medications that are ordinarily self‑administered.
- Assists with educational activities and habilitation, if indicated on the patient plan of care, to the extent allowable by the patient’s payer source.
- Assists select patients with homemaking/chores if indicated in plan of care and allowable by payer source.
- Documents all care given to patient in the patient medical record.
- Demonstrates the knowledge and skills necessary to provide care for the physical, psychological, social, educational, and safety needs of the patients served regardless of age.
- Duties as required.
- Communication skills are required, including the ability to read, write, and verbally report clinical information.
- Experience in Pediatrics and/or working with individuals with developmental delays is preferred.
- Experience working with all levels within an organization is required.
- Experience in healthcare is preferred.
- Proficiency in MS Office (Outlook, Excel, Word) or similar software is required.
- Education:
High School Diploma or equivalent is required. - Certification: CPR and First Aid certification is required. Home Health Aide certification is preferred, or the successful completion of a Home Health Aide training program (minimum of 75 hours classroom and supervised practical training) is required upon hire.
- Relevant experience: 0 to 2 years is preferred.
