Center Well Home Health is seeking a licensed Occupational Therapist to provide in-home rehabilitation, improving strength, mobility and independence for patients. You will work with physicians, nurses, and the care team to implement Plan of Care treatments and document progress.
Responsibilities include patient assessment, developing retraining programs, and facilitating discharge planning while educating patients and families. A valid OT license, CPR, and reliable transportation are required.
