Overview

Location:

Akron, OH. The Patient Care Liaison is a cross-functional healthcare support professional responsible for enhancing patient access, supporting medical workflows, assisting with continuity of care activities, and maintaining accurate documentation within a federally aligned primary care clinic. This position serves as a vital connection between patients, providers, clinical staff, and external healthcare organizations to ensure a seamless, patient-centered healthcare experience. The Patient Care Liaison combines administrative, patient service, and clinical support functions while helping patients navigate healthcare services and supporting providers in delivering high-quality, coordinated care.

Responsibilities
  • Welcome and assist patients in a professional, compassionate, and confidential manner.
  • Schedule, confirm, and coordinate patient appointments.
  • Verify demographic, insurance, and registration information.
  • Coordinate referrals to specialty providers and community resources.
  • Track referral status and obtain consultation reports.
  • Prepare patient charts and ensure documentation is complete prior to appointments.
  • Support providers with administrative and clinical workflow tasks.
  • Ensure patient information is documented accurately and timely.
  • Maintain confidentiality and comply with HIPAA regulations.
  • Serve as a liaison between patients, providers, behavioral health staff, nursing, care management, and external healthcare organizations.
  • Participate in team meetings and collaborate to improve patient outcomes.
  • Support clinic operations by assisting with special projects and other duties as assigned.
Qualifications
  • Minimum of 2 years’ experience in medical office operations, front desk registration, clinical support, or medical assisting.
  • High school diploma or equivalent required. Medical Assistant certification preferred.
  • Experience using electronic medical record systems.
  • Strong organizational and multitasking abilities with exceptional attention to detail.
  • Strong communication and customer service skills. Ability to work independently and collaboratively.
  • Understanding of patient confidentiality standards and HIPAA.
  • Ability to function effectively in a fast-paced, highly regulated healthcare environment.
