Position: Licensed Physical Therapy Assistant  PRN
Ohio Living Home Health -  - Responsibilities:
Implements the Plan of Care developed by the Physical Therapist, ensuring patients are treated per physician orders;
Communicates with the Physical Therapist, physician, and staff about physical/medical concerns pertaining to the Plan of Care;
Educates patients and families on exercise techniques, precautions, and self-care activities;
Performs home visits to implement the Plan of Care and guide patient in therapeutic activities;
Maintains clean, sanitary, and safe work areas and equipment; participates in interdisciplinary team meetings
