Our Participant Care Assistants play a vital role in supporting participants with personal care, engagement, and daily activities in our state-of-the-art day center.

Position SummaryThe Participant Care Assistant provides direct personal care and supportive services to PACE participants in the day center and clinic setting. This role assists participants with activities of daily living, promotes dignity and independence, and contributes to a safe, welcoming, and engaging environment.
Key Responsibilities- Assist participants with bathing, dressing, grooming, toileting, and mobility
- Provide support with meals, hydration, and feeding as needed
- Escort participants throughout the center and to appointments
- Monitor and report changes in participants' condition or behavior
- Assist with recreational and therapeutic activities
- Maintain a clean, safe, and organized environment
- Document care and observations accurately
- Collaborate with nurses, therapists, social workers, and the interdisciplinary team
- High school diploma or GED required
- 1 year experience working with older adults required
- Compassionate, dependable, and team-oriented
- Strong communication and observational skills
- Ability to safely assist with transfers and mobility
- Monday–Friday daytime schedule
- No nights, weekends, or major holidays
