Physical Therapist

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team
• Plan initial and subsequent treatment programs based on clinical findings
• Participate in clinical conferences to share and coordinate patients’ plans of care
• Treat residents one-on-one in private treatment sessions rather than group settings
• Use a functional-based treatment approach
• Individualize treatment sessions for senior patients
• Deliver personalized therapeutic and rehabilitative care within senior living communities
• Collaborate with rehabilitation therapists and care teams

• Degree in area of discipline
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment
• Excellent interpersonal, fiscal, and clinical skills
• Computer proficiency
