- Serve as the primary Legacy representative at the facility
- Promote Legacy policies, clinical programs, and the mission to help aging adults live safe, healthy lives
- Build rapport and positive working relationships with facility personnel and Legacy employees
- Oversee internal marketing efforts and keep community leaders informed about Legacy programs and offerings
- Manage site productivity and make changes to achieve budget
- Maintain a caseload to help the rehab department meet standards
- Assess patient needs and develop clinical programs appropriate for facility census
- Direct and train others in clinical service delivery across disciplines
- Coordinate the rehab department daily with facility staff to plan and implement therapy programming
- Participate in clinical outcomes reporting and share results with the customer
- Manage staff schedules to meet caseload requirements and use scheduling tools for patient schedules
- Manage Clini Fax and physician-signature requirements for therapy plans of care
- Maintain communication among rehab staff, facility staff, and Legacy corporate personnel
- Assist with staff orientation and training to meet company, state, and federal standards
- Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech Language Pathologist (SLP)
- Three to five years’ experience in long-term care (LTC) or senior living
- Minimum of a degree from an accredited college or university or equivalent experience, such as a degree in PT, PTA, OT, COTA, or SLP
- Licensed and registered as a PT, PTA, OT, COTA, or SLP in the state where assigned
- Prior supervisory experience preferred but not required
- Ability to assess patient needs and develop clinical programs
- Ability to direct and train others in clinical service delivery across disciplines
- Ability to manage productivity, budgets, caseloads, and staff schedules
- Ability to coordinate therapy programming with facility staff
- Ability to manage Clini Fax and physician signatures on therapy plans of care
- Ability to meet company, state, and federal standards
