Phoenix of Fairlawn is looking for a dependable and enthusiastic PRN Certified Occupational Therapy Assistant (COTA) to join our in-house rehabilitation team!

We have an immediate need for a COTA who can provide weekend support as needed. This is a great opportunity for a COTA looking to add consistent PRN hours while working with an experienced rehab team in a skilled nursing and long-term care setting.

• Provide skilled occupational therapy treatments under the direction of an Occupational Therapist

• Work with our SNF/LTC residents to improve independence, function, and quality of life

• Carry out individualized treatment plans and document resident progress

• Collaborate with OT, PT, SLP, nursing, and other members of the interdisciplinary team

• Complete timely and accurate documentation using Net Health

• Provide reliable weekend coverage for the rehab department

• PRN

• Weekend and Weekday coverage opportunities available

• Potential for additional PRN hours based on facility needs and availability

• Current Ohio COTA license

• Graduate of an accredited Occupational Therapy Assistant program

• SNF/LTC experience preferred

• Familiarity with Net Health is a plus

• Strong communication and teamwork skills

• Dependable and comfortable working independently when needed

• New graduates with an interest in SNF/LTC are welcome to apply
