Portage Lakes Nursing & Rehab is a tight-knit, community-focused healthcare campus featuring 56 Skilled Nursing beds and 15 Assisted Living beds. We are seeking an experienced, visionary Licensed Nursing Home Administrator (LNHA) to oversee our daily operations, lead our dedicated team, and foster a culture of clinical excellence and resident satisfaction.

• Operational Leadership: Direct and coordinate all daily operations across administrative, clinical, and support departments to maintain high standards of resident care and regulatory compliance.
• Regulatory & Survey Management: Serve as the primary contact for state licensing authorities, leading prep efforts and accompanying survey teams during facility inspections.
• Financial Management: Oversee departmental budgets, monitor financial performance reports, approve operational expenditures, and work closely with executive leadership to achieve fiscal goals.
• Team & Department Guidance: Mentor and evaluate department heads, oversee hiring and retention strategies, and maintain a supportive, transparent work environment.
• Quality & Safety Assurance: Ensure strict compliance with all local, state, and federal health, safety, fire, and sanitation codes. Direct facility committees including Quality Assurance, Infection Control, and Patient Care.
• Community & Resident Relations: Build strong relationships with residents, families, and community partners while serving as the primary ambassador for the facility.

• Licensure: Current, active Licensed Nursing Home Administrator (LNHA) license in the State of Ohio (Required).
• Experience: 2+ years of administrative leadership experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting required; experience overseeing Assisted Living beds preferred.
• Regulatory Knowledge: Deep understanding of Ohio Department of Health (ODH) regulations, CMS guidelines, and life safety codes.
• Leadership: Strong financial acumen, budget management skills, and a proven track record of staff mentorship and survey readiness.
