Summit County Public Health is seeking a Licensed Social Worker to join our Senior & Adult Services program, working full time (35 hours per week) to promote health equity for older adults in Summit County through wellness, prevention, healthcare access, and safety initiatives.
As an LSW, you will assess, plan, and implement strategies to enhance the health and independence of our older adult community members, including case management, BRI Care consultations, and outreach to caregivers.
